To be completed by the referring veterinarian. A completed outpatient CT request form is required prior to making an appointment for outpatient CT.
Once we receive your referral request, a MASH representative will contact the pet owner to schedule an appointment.
Patients referred for outpatient CT scans must be clinically stable outpatients and considered suitable for general anaesthesia and IV contrast.
Changes in patient status after admission to Melbourne Animal Specialist Hospital (MASH) may require transfer to our care.
Please Note:
- MASH will contact the client to schedule admission for CT upon receipt of the completed CT Request form.
- Admission to MASH will be via a Nurse admission appointment including completion of routine pre-anaesthetic safety checklist.
- Anaesthesia and recovery will be administered and supervised by MASH Nurses and Veterinarians.
- The pet owner is required to provide signed informed consent.
- An imaging report will be sent to the referring veterinarian within 2-3 business days and the owner will be advised to contact the referring veterinarian to discuss the imaging results.
- Laboratory test results must be provided including: haematology, biochemistry, urinalysis collected within the last 7 days.
- Pre-anaesthetic testing will otherwise be performed at MASH and invoiced to the client.
Service includes specialist imaging report, anaesthesia, IV contrast, IV fluids and hospitalisation.
